End-Of-Life Care

At Changing Tides, we believe that everyone deserves the option to spend their final days in comfort, surrounded by familiarity and love. You don’t need to move into a hospice or aged care facility if that’s not your wish. More individuals and families are choosing to receive end-of-life care at home—and we’re here to make that possible.

We provide in-home support tailored to your unique needs, with care delivered by a qualified Registered Nurse and experienced support staff. Our approach centres on your preferences and values, ensuring a calm, respectful, and nurturing environment.

As registered Death Doulas, we offer holistic care that supports not only the physical process of dying but also the emotional and spiritual journey. Our services are designed to feel personal, gentle, and non-institutional—because this chapter deserves to be met with dignity and compassion.

Respite Care

There is nothing more meaningful than caring for a loved one at the end of life, especially in the comfort of home. Being surrounded by familiar faces and a trusted environment can offer deep peace and connection during this sacred time.

But this care can also be physically, emotionally, and mentally demanding. No matter how loving, committed, or well-intentioned, carers can quickly become overwhelmed—especially without prior experience or support. Carer fatigue is real, and taking breaks is not only okay, it’s essential.

At Changing Tides, we’re here to support those who support others. Whether you need professional advice, hands-on assistance, or time to rest and tend to other responsibilities, we can step in with compassion and expertise.

Our team includes certified Death Doulas and a Registered Nurse, offering a wide range of support—from practical guidance and help with equipment hire to in-home care for a few hours, a full day, or even multiple days. Whatever your needs, we ensure that the care continues seamlessly and with the utmost respect for your loved one.

Ceremonies

Whether you're planning a funeral, memorial, or intimate celebration of life, we walk beside you with empathy and experience. As holistic death care practitioners and ceremonial guides, we help you craft rituals that are deeply personal, rooted in your values, stories, and beliefs.

End-Of-Life Wishes

Having the opportunity to express your end-of-life wishes is one of the most empowering and loving acts you can offer, to yourself and to those who care for you. At Changing Tides, we gently guide individuals and families through the process of clarifying, documenting, and communicating what truly matters in the final chapter of life.

This may include preferences for care, environment, rituals, spiritual or cultural needs, the kind of atmosphere you’d like in your final days, and how you wish to be remembered. We hold space for meaningful conversations that honour autonomy, clarity, and dignity by removing guesswork and easing the emotional weight on loved ones.

Legacy Projects

Everyone has a story worth telling—and remembering.

Legacy projects offer a meaningful way for people nearing the end of life to reflect on their journey, share wisdom, and leave something lasting for those they love. These projects can take many forms: letters, voice recordings, photo albums, recipe collections, handcrafted items, memory boxes, or even simple conversations recorded and preserved.

At Changing Tides, we support individuals and families in creating legacy pieces that feel authentic, heartfelt, and deeply personal. Whether guided or self-directed, these projects can bring comfort, connection, and a sense of purpose in the final stages of life.

Grief Education and Support

Grief is not something to be fixed, it is something to be felt, honoured, and supported. At Changing Tides, we offer grief education and compassionate guidance to help individuals, families, and communities navigate the complex emotions that follow loss.

Our approach is grounded in empathy, lived experience, and evidence-informed practices. We provide one-on-one grief companioning, small group circles, and tailored education sessions that explore the many faces of grief, from anticipatory grief to sudden loss, from collective mourning to the quiet ache that lingers.

Whether you're seeking support for yourself, your family, your workplace, or your community, our offerings help build understanding, connection, and resilience in the face of sorrow. Grief can be isolating, but it doesn't have to be endured alone.

Living Wake

We often celebrate someone’s life after they’ve passed—but what if that celebration could happen while they’re still here to be part of it?

A living wake gives people nearing the end of life the chance to take part in their own celebration. It’s a unique and powerful way to honour their life with them, on their own terms.

At Changing Tides, we believe everyone deserves the chance to say goodbye in a way that reflects who they are. Whether it’s a quiet gathering of close family, a beach picnic, a garden lunch, or even a day on a fishing trawler—your final celebration can be as unique as you are.

Emberlight Life Celebration

As a trained death doula, registered nurse, and Lifeline accredited Counsellor, Chris possesses a unique set of skills to provide comprehensive support to individuals facing the end of their life. Chris's dedication and passion for supporting individuals to die well with autonomy and a sense of empowerment is reflected in his core values; compassion and empathy, dignity, respect, presence, mindfulness and cultural sensitivity.

Jenny is a trained Death Doula and a children's and families empowerment advocate, with 25 years of professional experience supporting families. Jenny is described as authentic and grounded in her care, providing a calm and stable presence in her work. She is passionate in developing rich and genuine connections through holistic and person-centred care.
